Help! I need a replacement Diary/CP List/Handbook!

I've used up all the pages in my Diary...
The neighbour's dog ate my Handbook...
My CP List caught fire while I was trying to make the latest Masterchef recipe...

If you require a replacement DAFNE diary, CP List, or Handbook, first try your local DAFNE team as they often have copies they can sell to you.

If your local DAFNE team is all out of stock then please request an order form from the OzDAFNE Project Officer and email, fax or post the completed form along with your payment as per the instructions on the form. Payment must be made by credit card, cheque or money order; no cash payments will be accepted. Please note: postage charges will apply.

Alternatively, if you are in Victoria then you can pick up new DAFNE diaries by visiting the Diabetes Australia - Vic Retail Shop, located at 570 Elizabeth Street, Melbourne. The Retail Shop accepts all forms of payment including cash.
